FACT Inspector Application Process

The following steps outline how to apply to become a FACT Inspector. You will be asked to gather specific documents and complete forms in electronic format, which will be uploaded within the online Inspector Application.

Gather Documents

Documents must be in Word or PDF
Current curriculum vitae or resume
For clinicians only: Medical license verification link or electronic copy of current medical license and professional liability claim/suit information

Complete Application

This application will take less than ten minutes of your time. Once you start the application, you must finish it. You cannot return to edit your inspector application.
You will be prompted to login or obtain a password. If you already have a FACT username/password, you will be able to use your login credentials; otherwise, you will have an opportunity to create a FACT profile.
When creating or updating your FACT profile, you will enter or update your name, addresses, and contact information
You will be directed to continue the FACT Inspector Application

Complete Annual Compliance

Once your completed application has been reviewed you will be sent a link to complete Annual Compliance.
Ensure your documents are ready to upload prior to starting Annual Compliance.
